Sorry for the gloomy title but I think these type of scenes are just so powerful... 
Haven't watched a lot of old school animes so I'll just focus on the most recent animes I've watched so far. Here's what I think are the most powerful anime death scenes...
1) Macross Frontier Episode 20 - Death of Michel
Setting: Vajra infiltrates Frontier from the inside... amidst the chaotic city, Klan and Michel confide their feelings for each other in the most dramatic way...
Cause of death: Stabbed by a Vajra
Why I liked it: I know it's the typical say-what-you-feel-before-you-die scene. But aside from the fact that I was surprised that Michel died first instead of Luca or Ozma, the background music (Diamond Crevasse) was just the perfect accompaniment to the scene. The phasing of the music (when they paused the song during the actual stabbing) reflects the phasing of the viewers hearbeat (unless of course if you didn't liked the series nor the scene). And one last thing, I just can't forget the scene where Alto tried to pull Michel from the vacuum caused by the Vajra's explosion.
2) Code Geass R2 Episode 13 - Death of Shirley
Setting: After regaining her memory, Shirley pursues Lelouch hoping to join him in his "battle." Until he met Lelouch's fake brother, Rolo...
Cause of death: Shot by Rolo
Why I liked it: It reminded of Innoue's confession to Ichigo where she said that in any lifetime, she would still like the same guy... I know it's kinda mushy but hell I think it's very sweet... But what really struck me was the way Lelouch desperately used his Geass to try to save her... continuously casting his geass through his teary eyed... I really felt sorry for the guy...
